Key 2022 Forms

Our forms are designed to give us all the information we need to prepare for your child’s arrival at camp. We know that they take some time to complete but you should feel confidant knowing that you wont forget to provide us with important details about your child if your forms are fully completed. The following 3 forms must be completed well before your child’s first day at camp. If you have not done so already, please do so ASAP. Note: When completing the Health Form, simply use the same login information you used when completing the application.
